Before your appointment

Email us a photo of the front and back of your insurance card.

Get CD copies of your X-Rays, MRIs, and CTs from the imaging facility.

Do not allow the facility to mail the CD as this takes weeks to reach us.

What to bring to your appointment

Any imaging, studies, and medical records that pertain to your injury.

X-Rays, MRIs, CT scans, EMG studies, Ultrasounds, procedure and surgery reports, etc.

Your insurance card if you did not email us a photo.

A driver if you plan on getting a procedure.  See pre-op instruction below.

If you do not know, call or email our office and we will let you know.

Procedure Pre-Op Instructions
No eating or drinking 6 hours prior to your scheduled appointment.

You may take medications with a sip of water. If you are on a blood thinner call our office.

You are required to bring a driver who must remain in our office throughout the entire appointment.

This is MANDATORY. If your driver is not with you, your appointment will be rescheduled.

If you are sick (cold, flu, etc.) or are taking antibiotics, we will need to reschedule your procedure. Call or email our office.

Stop taking medications such as:

Motrin, Aleve, Aspirin, Ibuprofen, or any other NSAIDS. Continue all other medications.

Procedure Post-Op Instructions
No showering or driving until the next morning.

You may eat and drink right after your appointment.

No exercising, heavy lifting, or exerting yourself for 1 week.

Healing doesn't happen on the table, it happens at home.

If you need a work note or temporary profile email or call our office.

Follow up one week after your procedure.

Your appointment is at the bottom of your post-op sheet. If you don't know, call or email our office.

If you feel you need emergent care go to the Emergency Room or call 911.
